August 14th 2026 is the registration deadline. Please ensure all required documents and payment are received on or before this date.

By signing this document, I understand that…
– No refunds will be granted after the third night of classes; textbooks issued must be returned.
– I must attend 150 hours of classroom instruction to successfully complete this program year.
– Classes are held Monday and Wednesday evening fromm 5:30 PM – 9:00 PM
– Classes may be canceled up to 3:00 PM before a class due to inclement weather (or equal reason), and I will be notified by the contact information I provide with this form.
– I must take a minimum of ten tests and receive a grade average of 70% or higher to successfully complete this program year.
– The program must have a minimum of six (6) students in a class to be conducted, and I will be notified should the class I’m registering for fall below that number before the start of the program term.

I authorize the Manchester Apprenticeship Program to release academic progress in apprenticeship-related instruction (i.e., attendance records and grades) to my employer or sponsor, the US DOL Office of Apprenticeship, or the State Licensing Board. Additionally, if I am registered with an out-of-state registration agency, I authorize the release of my information to that agency. I understand that released information will be used ONLY to monitor and evaluate my progress in the apprenticeship program and for no other purpose. I understand that no information will be released to any other person or organization without my written consent. This release is in conformity with the Privacy Act of 1974.
